Each student registering for dance classes will receive a free T-shirt.  Please circle size:

List below any days or times that are impossible for class attendance.  Every effort will be made to accommodate your schedule.  You will be confirmed of your child’s class day and time by mid-July. (In the event that we are unable to accommodate you, your registration fee will be refunded.)

I, as a parent or legal guardian, do not hold Steps of Faith Dance Studio, Steps of Faith Dance Company, Julie Reeves or other instructors of Steps of Faith, responsible for any injury the student obtains on the premises of Steps of Faith, other class or performance location.  I hereby release Steps of Faith and Julie Reeves and/or its instructors of any legal liability.
